The notice will provide the tax type from which the overpayment originated and tax type to which it is being applied.You must include your name, your SSN, and your mailing address. When claims are adjusted due to an overpayment, a Solicited Demand letter will be issued to you. (1) The Office of the Comptroller will issue a cost settlement letter to the provider notifying the provider of the amount of the overpayment. The letter notifies the individual of the amount of the overpayment claim and requests full repayment. The individual may choose the type of payment option.
