Description
An Affidavit of Domicile is required for transferring stock certificates when the registered owner of the stock is deceased. This document will confirm the state of residence at the time of death for tax purposes. The Affidavit of Domicile must be signed by the individual executor, administrator and/or survivor in the presence of a Notary Public. You must have lived in New York State for 12 months prior to the start of classes. You must upload proof of residency showing your name, date (one year prior to the start of classes) and your current address.A student will not be admitted into the school system unless they are a resident of the Town of Franklin and presents "proof of residency". The certificate of residency obtained from your home county is the only proof of residence that qualifies for resident or in-state tuition.
