Proof of residency is required of all first year and renewal applicants and must be submitted to the provider with the application. Such students wishing to enroll at a school with the District must provide proof of residency as contained in Division 3 above. b.An Affidavit of Residency (PDF) must be completed in the presence of a Notary Public. 1. The Residency Affidavit (page 2) must filled out completely and signed before a notary public. Proof of residency is required for all newly enrolled students and any student whose address changes. One item from list below will serve as proof of residency.
