This form is used by a Landlord to terminate a residential Month-to-Month Lease due to Tenant's breach of the lease agreement, for a breach other than non-payment of rent. "Residential" includes a house, apartment or condo. "Month-to-Month Lease" is a periodic lease that continues from month to month until either Landlord or tenant decides to terminate, or it is terminated for cause (as in the case of a breach of the lease). The form identifies the specific breach and directs the Tenant to vacate the premises within 14 days of receipt of the Notice. No chance for the Tenant to cure (repair) the breach is required by state law, but the Landlord may elect to allow Tenant a chance to cure by indicating so in the check-box provided in the form.
