The Arizona Cremation Certificate is a legal document that is used to certify that a cremation has been performed in accordance with the laws of the State of Arizona. It is a document provided by the crematory to the family and/or the funeral director of the deceased. It includes information such as the name of the deceased, the date of the cremation, the name of the crematory, the name of the person authorizing the cremation, and the method of disposition (e.g., burial, scattering of ashes, etc.). The Arizona Cremation Certificate is also used to provide proof of cremation to insurance companies, Social Security Administration, and other government agencies. There are two types of Arizona Cremation Certificates: Form A and Form B. Form A is used for the disposition of cremated remains and Form B is used for the disposition of a casketed body. Both forms must be signed by the person authorizing the cremation.
