The Alaska Notice of Transfer of Supervision is a document used by the Alaska Department of Corrections when transferring a person from one form of supervision to another. This document is used to transfer a person from probation to parole, or vice versa, or from a halfway house to supervision in the community, or from one institution to another. It also serves to inform the receiving entity of the transfer and the details of the person being transferred. There are two types of Alaska Notice of Transfer of Supervision: the Standardized Notice of Transfer of Supervision, and the Specialized Notice of Transfer of Supervision. The Standardized Notice of Transfer of Supervision is used for all transfers of supervision, regardless of the type of supervision being transferred or the receiving entity. The Specialized Notice of Transfer of Supervision is used when the receiving entity or the type of supervision being transferred requires additional information.
