Notice of Absence from Treatment Facility, is an official form from the Alaska Court System, which complies with all applicable laws and statutes. USLF amends and updates the forms as is required by Alaska statutes and law.
The Alaska Notice of Absence from Treatment Facility is a form used to document and report the absence of an individual from a treatment facility in the state of Alaska. It is used to notify the facility's governing body and to document the individual's absence, including any mitigating circumstances and/or details regarding the individual's whereabouts. There are two types of Alaska Notice of Absence from Treatment Facility: 1) Voluntary Absence; and 2) Involuntary Absence. A Voluntary Absence is where an individual has chosen to absent themselves from the facility without explicit permission or notification. An Involuntary Absence is where an individual has been absent from the facility without permission, and an investigation may be necessary to determine the individual’s whereabouts.
