The Alaska Cover sheet is a document used by Alaskan businesses to report wages and other taxable income to the state's Department of Revenue. This document is filed with the Alaska Department of Revenue along with the Alaska Employer's Quarterly Contribution and Wage Report. There are three types of Alaska Cover sheet: the Alaska Small Employer's Contribution and Wage Report, the Alaska Employer's Quarterly Contribution and Wage Report, and the Alaska Employer's Annual Contribution and Wage Report. The Alaska Small Employer's Contribution and Wage Report is used by employers with fewer than 50 employees to report wages and taxes. The Alaska Employer's Quarterly Contribution and Wage Report is used by employers with more than 50 employees to report wages and taxes each quarter. The Alaska Employer's Annual Contribution and Wage Report is used by employers to report wages and taxes on an annual basis.
