The Alaska Victim Notice of Change of Contact Information is a form issued by the Alaska Department of Corrections (DOC) that allows victims to update their contact information on the DOC’s Victim NotificatioSystemsNS). It is used to ensure that victims are notified in the event of a change in the status of an offender, such as release, transfer, or escape. There are three types of Alaska Victim Notice of Change of Contact Information forms: one for a single victim, one for multiple victims, and one for victims of crime-related financial losses. The single victim form requires information about the victim’s name, address, phone number, and email address. The multiple victims form requires the same information for each victim. The form for victims of crime-related financial losses requires information about the victim's name, address, phone number, and the amount of the loss. All forms must be completed and returned to the DOC in order for the VS to be updated.
