The Alaska Controversial (Denial) Notice is a form issued by the State of Alaska to notify a claimant of the denial of a claim for workers' compensation benefits. There are three types of Alaska Controversial (Denial) Notice: 1) Initial Notice of Controversial, 2) Final Notice of Controversial, and 3) Notice of Controversial After Settlement. The Initial Notice of Controversial is issued when an employer or insurance company has denied a claim or controverted the claim. The Final Notice of Controversial is issued after a hearing decision has been made or the claim has been administratively denied. The Notice of Controversial After Settlement is issued after a settlement agreement has been reached. The Notice of Controversial contains the reason for the denial, the right to appeal the decision, and instructions on how to appeal.
