The Alaska Notice of Transfer of Supervision is a form used by the Alaska Department of Corrections to transfer the supervision of an individual from one probation or parole officer to another. There are two types of Alaska Notice of Transfer of Supervision: Supervision Transfer of Offender and Supervision Transfer of Juvenile. The form includes the name and address of the offender, the name and address of the new probation or parole officer, the date of the transfer, and any special conditions or instructions given to the new officer. The form must be signed by the original probation or parole officer and the new probation or parole officer.
