This form is required if a person is looking to change their name due to naturalization or a court order. Both forms require the applicant to provide their current name, the desired name, the reason for the change, and other information. Once the forms are completed and notarized, they must be filed with the appropriate Alaska agency, such as the Division of Motor Vehicles.
