The Alaska Statement of Change (Domestic Professional Corporation) is a document that is used to inform the Alaska Department of Commerce, Community, and Economic Development of changes to a domestic professional corporation. This document must be filed with the Division of Corporations, Business and Professional Licensing if a corporation wishes to change its name, amend its Articles of Incorporation, or add or remove officers or directors. There are three types of Alaska Statement of Change (Domestic Professional Corporation): Name Change, Amendment, and Officers/Directors Change. The Name Change form is used to change the name of a domestic professional corporation, while the Amendment form is used to make amendments to the Articles of Incorporation. The Officers/Directors Change form is used to add or remove officers or directors from the corporation. All forms must be signed by a corporate officer and must be accompanied by the applicable filing fee.
