The Alaska Notice of Change of Officials (Domestic Electric or Telephone Cooperatives) is a document that is required by the state of Alaska in order to notify the state of any changes in officers, directors, or other key personnel at domestic electric or telephone cooperatives. This document must be signed by the current president of the cooperative and filed with the Alaska Division of Corporations, Business and Professional Licensing. There are two different types of Alaska Notice of Change of Officials documents that can be filed: one for Domestic Electric Cooperatives and one for Telephone Cooperatives. Both documents must include the name and address of the cooperative, the name of the new officer or director, the date of the change, and the signature of the current president of the cooperative. The documents must be submitted to the Division of Corporations, Business and Professional Licensing within 30 days of the change.
