Alaska Motion & Affidavit to Modify Custody, Visitation and/or Child Support, SHC-1500 is a court form used in the state of Alaska to request a modification of child custody, visitation and/or child support. The form can be used to modify existing court orders or to propose new orders. The form is used for both parents and third parties who are seeking to modify existing orders.
