Alaska Instructions for Motion to Modify Custody, Visitation and/or Child Support, SHC-1502, is a form that must be filled out by the person filing a motion to modify an existing court order relating to child custody, visitation, or child support. This form is used in all family law cases in Alaska. It includes information such as the names of the parties involved, a statement of facts, proposed modification, and a proposed order. There are two types of Alaska Instructions for Motion to Modify Custody, Visitation and/or Child Support, SHC-1502: (1) for modification of custody and visitation; and (2) for modification of child support. Both forms must be completed and filed with the court in order for an existing court order to be modified.
