Alabama Form 4: Psychological Disability Verification is a document used to confirm a person's psychological disability status and to provide proof of their disability to employers or other organizations. This form is required by the Alabama Department of Rehabilitation Services (ADDS) in order to receive services such as vocational rehabilitation or independent living services. The form must be completed by a licensed psychologist or other qualified mental health professional and must include details about the individual's diagnosis, symptoms, functional limitations, and treatment plan. There are three types of Alabama Form 4: Psychological Disability Verification: Type I, Type II, and Type III. Type I is used for individuals with severe mental illness, Type II is used for individuals with a milder form of mental illness, and Type III is used for individuals with a psychological disability not covered by Types I or II.
