Alabama Form 5: Visual Disability Verification is a form provided by the Alabama Department of Rehabilitation Services (ADDS) to verify a person’s visual disability. This form is used to verify a person’s eligibility for Vocational Rehabilitation Services, Blind Services, or other services provided by ADDS. It also serves as proof of a visual disability in order to obtain benefits from the Social Security Administration. There are two types of Alabama Form 5: Visual Disability Verification: a Short Form and a Long Form. The Short Form is used to certify a visual disability and is completed by a professional eye care provider. The Long Form is used to certify a visual disability and also includes an assessment of the individual’s functional abilities related to the visual disability, and is completed by a qualified vision rehabilitation specialist.
