The Alabama Valid Court Order (VCO) Exception Assessment Request Checklist is used by Alabama Medicaid to assess requests for exceptions to the valid court order (VCO) requirement for certain long-term care services. This checklist is used to help ensure that all applicable criteria have been met before an exception is granted. It includes two types of checklist: 1) VCO Exception Assessment Request Checklist — Adult and 2) VCO Exception Assessment Request Checklist — Child. The Adult VCO Exception Assessment Request Checklist is used to assess requests from adults and includes the following criteria: 1) Verification that the individual is an adult; 2) Documentation of the individual’s need for long-term care services; 3) Verification that a valid court order (VCO) is not available; 4) Documentation to show that the individual has made reasonable efforts to obtain a valid court order; and 5) Documentation of the individual’s financial resources and/or income. The Child VCO Exception Assessment Request Checklist is used to assess requests from children and includes the following criteria: 1) Verification that the individual is a minor; 2) Documentation of the individual’s need for long-term care services; 3) Verification that a valid court order (VCO) is not available; 4) Documentation to show that the individual has made reasonable efforts to obtain a valid court order; and 5) Documentation of the legal guardian’s financial resources and/or income. These checklists are used to assess requests for exceptions to the valid court order (VCO) requirement for certain long-term care services in Alabama.
