The Arkansas One-Time Credit Card Authorization Form is a document used by merchants and businesses in Arkansas to authorize a single credit card transaction. This form must be completed and signed by the cardholder in order to process the transaction. It is also known as a One-Time Credit Card Authorization Form, or a One-Time Credit Card Payment Authorization Form. The form includes the cardholder's name, address, and contact information; the credit card number and expiration date; the amount of the transaction; and the cardholder's signature. There are two types of Arkansas One-Time Credit Card Authorization Forms: the Regular Arkansas One-Time Credit Card Authorization Form and the Arkansas One-Time Credit Card Authorization Form for Non-Merchant Transactions. The Regular Arkansas One-Time Credit Card Authorization Form includes additional information such as the merchant's name and address, the transaction date, and the cardholder's authorization to charge the transaction. The Arkansas One-Time Credit Card Authorization Form for Non-Merchant Transactions is used for transactions that do not involve a merchant, such as online payments. This form requires the cardholder's name, address, and contact information; the credit card number and expiration date; the transaction amount; and the cardholder's signature.
