The Arkansas Health & Safety Plan Cover Sheet is a document that is required to be completed by employers in the state of Arkansas that have safety plans in place. It includes information about the employer and their safety plan, and serves as a cover to the plan itself. The cover sheet must include the name of the employer, the address of the workplace, the safety plan title, the date of the safety plan, the contact person, and the signature of the employer or authorized representative. The cover sheet must also include a brief summary of the safety plan, and a list of all safety protocols that are included in the plan. There are two types of Arkansas Health & Safety Plan Cover Sheet: the Standard Arkansas Health & Safety Plan Cover Sheet, and the Enhanced Arkansas Health & Safety Plan Cover Sheet. The Standard Arkansas Health & Safety Plan Cover Sheet is a basic outline of the employer’s safety plan and is submitted to the Arkansas Department of Labor. The Enhanced Arkansas Health & Safety Plan Cover Sheet is a more comprehensive and in-depth document which includes additional information about the employer’s safety plan, such as a detailed description of the safety protocols that are included in the plan.
