Notice to Employer: This form is used by employers to inform employees of changes in the terms and conditions of their employment. This includes changes to wages, hours, job duties, and location. Notice to Employee: This form is used by employers to inform employees of changes in their employment status, such as termination, demotion, or job relocation.
