The Notice of Change of Address — Arizona Supreme Court is a document that informs the Arizona Supreme Court of a change of address for the petitioner or respondent. This notice must be filed with the Clerk of the Supreme Court in order to keep the Court informed of the petitioner's or respondent's current address. There are three types of Notice of Change of Address — Arizona Supreme Court: 1) Notice of Change of Address — Petitioner; 2) Notice of Change of Address — Respondent; and 3) Joint Notice of Change of Address. All notices must include the case name and number, the name and address of the petitioner or respondent, and the new address. The notice must be signed by the petitioner or respondent or their attorney. The notice must also be accompanied by a copy of a valid government-issued identification, such as a driver's license or passport. The Notice of Change of Address — Arizona Supreme Court must be filed with the Clerk of the Supreme Court within 10 days of the change of address.
