Arizona Unclaimed Funds Application — Amended is a form used by the Arizona Department of Revenue to allow individuals, businesses, and organizations to apply for unclaimed funds held by the state of Arizona. The application can be used to claim money from unclaimed estates, unclaimed bank accounts, unclaimed insurance policies, unclaimed wages, and other sources of unclaimed funds. It can also be used to claim funds from estates of individuals who have passed away. There are three types of Arizona Unclaimed Funds Application — Amended: Unclaimed Property Application, Unclaimed Property Claimant Representation Authorization, and Unclaimed Property Affidavit. The Unclaimed Property Application is used to search for and claim unclaimed funds. The Unclaimed Property Claimant Representation Authorization is used to authorize a representative to search for and claim unclaimed funds on behalf of an individual or business. The Unclaimed Property Affidavit is used to claim unclaimed funds from estates of individuals who have passed away.
