The Arizona Statement of Military Service is a document issued by the Arizona Department of Veterans' Services (AZ DVS) that provides proof of a person's honorable military service. It is a legal document that can be used to obtain benefits, services, and privileges that are available to veterans in the state of Arizona. There are two types of Arizona Statement of Military Service: the Certificate of Completion of Military Service (CCS) and the Arizona Veteran Service Certificate (ASC). The CCS is issued to veterans who have completed active duty in the United States Armed Forces and have been honorably discharged or released under honorable conditions. The ASC is issued to veterans who have been honorably discharged from the Arizona National Guard or from the United States Armed Forces Reserve. Both documents provide proof of military service and are accepted as a valid form of identification at many government agencies and businesses. The AZ DVS also issues duplicate copies of the CCS and ASC for a fee.
