The Arizona Authorization and Voucher for Payment of Transcript (AV PPT) is a form issued by the Arizona Department of Education (ARE) to authorize payment for a transcript from a college or university. The AV PPT is used when a student or former student needs an official transcript from the school in order to receive credit for coursework or to transfer to another school. The AV PPT is completed by the student or former student and then sent to the school for processing. There are two types of AV PPTs: one for current students and one for former students. The AV PPT for current students must be completed by the student's school, and the AV PPT for former students must be completed by the former student. Both forms require the student or former student to provide information such as the name and address of the school, the student's name and contact information, and the type of transcript requested.
