Arizona Complaint for Employment Discrimination is a form used by an employee to file a complaint for discrimination in the workplace. The complaint is filed with the Arizona Civil Rights Division of the Arizona Attorney General's Office. The complaint form is used to document allegations of discrimination based on a person's race, color, national origin, sex, religion, age, disability, or genetic information. There are two types of Arizona Complaint for Employment Discrimination — the Administrative Complaint and the Judicial Complaint. The Administrative Complaint is a form used to register a complaint with the Arizona Civil Rights Division of the Arizona Attorney General's Office. The Judicial Complaint is a form used to file a formal lawsuit in the Arizona Superior Court. Both types of complaint require the employee to provide specific information about the alleged discrimination, including the date it occurred, the name of the employer or other responsible individual, and a detailed description of the discriminatory action.
