The Arizona Application to Proceed in District Court Without Prepaying Fees or Costs is a legal document used by individuals who cannot afford to pay court costs and fees associated with filing a case in district court. This application can be used by individuals filing civil or criminal cases in Arizona courts. It is also known as an formma pauperis” (IFP) application. The Arizona Application to Proceed in District Court Without Prepaying Fees or Costs is composed of two parts: Part I is the Application Form and Part II is the Affidavit. Part I requests basic information about the applicant, including name, address, and telephone number. It also requires the applicant to provide the name of the court in which the case is being filed, the case number, the party or parties involved, and a brief description of the case. Part II of the application requires the applicant to provide a sworn statement, verifying their inability to pay the filing fees and costs associated with the case. In Arizona, there are two types of Application to Proceed in District Court Without Prepaying Fees or Costs: a “Petitioner’s Affidavit” and a “Compliance Affidavit.” The Petitioner’s Affidavit is used by individuals filing a case in district court.
