The Arizona Record of the Number of Grand Jurors Concurring in an Indictment is a legal document that is filed with the court after a grand jury has considered the evidence and voted to indict a defendant. The purpose of the Record is to provide a record of the number of grand jurors who voted to indict the defendant, as well as the number of those who voted not to indict. The Record is commonly referred to as an “indictment sheet” and is maintained by the court clerk. There are two types of Arizona Record of the Number of Grand Jurors Concurring in an Indictment: the single-count indictment and the multiple-count indictment. For a single-count indictment, the Record will include the date of the indictment, the name of the defendant, the name of the grand jury, the county in which the indictment was returned, the name of the charging document, and the signatures of the grand jurors who voted to indict the defendant. For a multiple-count indictment, the Record will include the same information as a single-count indictment, as well as a list of all the counts in the indictment and the number of grand jurors who voted to indict on each count.
