Arizona Order Of Voluntary Paternity is an Arizona court order that establishes the legal father of a child when the parents are not married. This order is used to legally establish paternity and provides the father with all the rights and responsibilities associated with being the legal father of the child. It includes the father's name on the birth certificate, rights to visitation and custody of the child, and the responsibility to financially support the child. There are two types of Arizona Order Of Voluntary Paternity: Acknowledgement of Paternity and Administrative Establishment of Paternity. The Acknowledgement of Paternity is completed by the father voluntarily signing the form and is then filed with the Arizona Department of Health Services. The Administrative Establishment of Paternity is done through a court order and requires a court hearing.
