Health Professionals Report
The Arizona Health Professionals Report (APR) is a centralized database of all health professionals licensed, certified, and/or registered in the state of Arizona. This report is available to the public, employers, and other state agencies to verify the credentials of any health professional in the state. It is administered by the Arizona Department of Health Services (ADDS) and updated regularly to ensure accuracy and currency. The APR contains information on more than 240,000 licensed, certified, and/or registered health professionals in Arizona, including physicians, nurses, pharmacists, dentists, optometrists, physical therapists, and massage therapists. It also includes information on disciplinary actions, licensure requirements, and other relevant data. The APR is divided into four main categories: Licensing, Certification, Registration, and Disciplinary Actions. Each category can be searched by name, license or certification number, and other criteria. The APR also offers additional resources such as publications, forms, and frequently asked questions.
