Arizona Consent of Parent to Guardianship and/or Conservatorship of Minor Child and Waiver of Notice is a legal document that must be signed by a parent or guardian of a minor child in the state of Arizona in order to give consent to another adult to become a legal guardian or conservator of their child. This document waives the parent’s rights to receive notice of court proceedings related to the guardianship or conservatorship. There are two types of Arizona Consent of Parent to Guardianship and/or Conservatorship of Minor Child and Waiver of Notice documents. The first is for guardianship only, and the second is for guardianship and conservatorship. Both documents require the parent or guardian to provide information such as the full name of the minor child, the name of the proposed guardian or conservator, and the parent’s or guardian’s contact information. The parent or guardian must also acknowledge that they understand the rights and responsibilities of the guardian or conservator, and agree to waive their right to notice of any court proceedings related to the guardianship or conservatorship.
