An Arizona Order to Guardian(s) and Conservator(s) and Acknowledgement is a legal document that is issued by a court in Arizona. It appoints one or more people to serve as the legal guardian and conservator for a minor or an incapacitated person. The order outlines the duties and responsibilities of the guardian and conservator, as well as the rights of the person for whom the guardianship and conservatorship is being created. There are two types of Arizona Order to Guardian(s) and Conservator(s) and Acknowledgement: 1. Minor Guardianship/Conservatorship Order and Acknowledgement: This document is used to appoint a guardian and conservator for a minor under the age of 18. 2. Adult Guardianship/Conservatorship Order and Acknowledgement: This document is used to appoint a guardian and conservator for an adult over the age of 18 who is deemed incapacitated due to physical or mental disability.
