The Arizona Consent of Parent to Guardian and/or Conservator of a Minor Child and Waiver of Notice is a legal document used to give permission for a guardian or conservator to take full control over a minor child. This document is typically used when a parent or legal guardian is temporarily unable to take care of the minor child, or when the court has determined that it is in the best interest of the minor child to appoint a guardian or conservator. This document must be signed by both the parent(s) or legal guardian and the proposed guardian or conservator. There are two types of Arizona Consent of Parent to Guardian and/or Conservator of a Minor Child and Waiver of Notice: Limited Consent and Permanent Consent. Limited Consent allows the guardian or conservator to take temporary control of the minor child, such as for a vacation or medical treatment. Permanent Consent gives the guardian or conservator full control over the minor child until the minor child reaches the age of majority. Both Limited and Permanent Consent documents must be notarized and filed with the court in order to be legally binding. The document must also include specific information such as the names of the parent(s) or legal guardian, the proposed guardian or conservator, the relationship between the parent(s) or legal guardian and the proposed guardian or conservator, and a waiver of notice of the appointment of the guardian or conservator.
