The Arizona Petition for Termination of Guardianship and/or Conservatorship of Minor and/or Release of Restricted Funds is a legal document that is filed in the Superior Court of Arizona in order to end the guardianship or conservatorship of a minor. This document also requests that any funds that were held in a restricted account on behalf of the minor be released to the minor or to another designated party. There are two types of this petition: one for termination of guardianship and conservatorship and another for release of restricted funds. In the first type, the petitioner must provide evidence that the guardianship or conservatorship is no longer needed and that the minor is able to manage his/her own affairs. In the second type, the petitioner must provide evidence that the minor is entitled to receive the funds and that the funds are no longer needed for the purpose for which they were originally intended. Both types of petitions must be signed by the petitioner, the minor, and a witness.
