The Arizona Affidavit of Service by Certified Mail is a document used to prove that a legal document has been served to a specified individual. This document is typically used when service of process is being done by certified mail. It is an affidavit sworn to by a third party individual, such as a postal worker or process server, that the document was served by certified mail. The affidavit contains details about the individual served, the service address, the name and address of the sender, and the date and time of delivery. There are two types of Arizona Affidavit of Service by Certified Mail- the original Affidavit of Service by Certified Mail, and the Return Receipt Affidavit of Service by Certified Mail. The original Affidavit of Service by Certified Mail is used to prove that the document was served to the correct address and that the individual was properly notified. The Return Receipt Affidavit of Service by Certified Mail is an additional affidavit that is used to prove that the document was received by the individual. It is signed by the recipient upon receipt and contains the date and time the document was received.
