The Arizona Affidavit Regarding Minor Children is a document used to provide information about minor children in the event that a parent needs to be absent or cannot be present. This document is completed by the parent or guardian of the minor children and is used to designate another person to take care of the children as well as provide contact information in case of an emergency. There are two types of Arizona Affidavit Regarding Minor Children: a General Power of Attorney and a Limited Power of Attorney. The General Power of Attorney is used to give someone the authority to make decisions on behalf of the parent or guardian in the event of their absence, such as medical decisions, educational decisions, and other decisions related to the minor children. This document must be notarized and signed by the parent or guardian. The Limited Power of Attorney is used to give another person the authority to make decisions related to the minor children in the event of the parent or guardian’s absence. This document does not give the person the power to make medical decisions, but does provide the person with the power to make decisions related to the minor children’s education and other decisions related to the minor children. This document must also be notarized and signed by the parent or guardian.
