The Arizona Appointment of a Permanent Guardian and/or Conservator for a Minor — Part 1 (Table of Forms) is a set of documents used to appoint a legal guardian and/or conservator for a minor. In the state of Arizona, a minor is defined as any person under the age of 18. The forms included in the Table of Forms includes: Petition for Appointment of Permanent Guardian and/or Conservator, Verification of Petition, Notice of Hearing, Order Appointing Permanent Guardian and/or Conservator, and Bond of Guardian and/or Conservator. These forms must be completed and submitted to the court for the appointment of a permanent guardian and/or conservator. The Petition for Appointment of Permanent Guardian and/or Conservator must include the proposed guardian and/or conservator's name, address, and contact information, as well as the minor's name, address, and contact information. Additionally, the Petition must include a statement of why the appointment is necessary and why the proposed guardian and/or conservator is the best choice for the minor. The Verification of Petition is then used to verify the accuracy of the information provided in the Petition. The Notice of Hearing is then sent to the proposed guardian and/or conservator, informing them of the hearing date and time. The Order Appointing Permanent Guardian and/or Conservator is then issued by the court, officially appointing the proposed guardian and/or conservator and detailing their duties and responsibilities. Finally, the Bond of Guardian and/or Conservator is used to ensure that the guardian and/or conservator will fulfill their responsibilities and duties as appointed by the court.
