The Arizona Affidavit Of Person To Be Appointed Guardian And/Or Conservator is a legal document to appoint a guardian and/or conservator for an individual who is unable to care for themselves due to mental or physical limitations. This document is used to protect the individual’s financial and personal interests, and to ensure their wellbeing. It is usually used when someone is elderly, disabled, or otherwise incapacitated. There are three types of Arizona Affidavit Of Person To Be Appointed Guardian And/Or Conservator: 1.
