Arizona Appointment of a Permanent Guardian and/or Conservator for a Minor — Part 2 (Table of Forms) is a set of forms used by the court system to appoint a permanent guardian and/or conservator for a minor in the state of Arizona. The forms are used to document the appointment process, which includes appointing a guardian and/or conservator, and creating a plan for the minor's care. The forms are divided into four categories: Appointment of Permanent Guardian and/or Conservator, Guardian and Conservator Qualification and Bond, Inventory and Accounts, and Final Order of Appointment. Each form serves a different purpose in the appointment process. The Appointment of Permanent Guardian and/or Conservator form is used to appoint a guardian and/or conservator and establish their authority.
