The Arizona Child Support Financial Affidavit (also known as a Form 100A) is a document used in child support cases in the state of Arizona. It is used to provide the court and other parties with financial information to assist in the determination of the amount of child support. This form includes information on both parties’ incomes, assets, liabilities, medical expenses, and other relevant financial details. There are two types of Arizona Child Support Financial Affidavits: the Short Form and the Long Form. The Short Form is used in cases where both parties are in agreement regarding the amount of child support. The Long Form is used in cases where the parties are not in agreement or where there are additional financial matters to consider.
