Arizona General Order to Guardian and Acknowledgement is a legal document completed when a person is appointed to serve as guardian of a minor or incapacitated adult. It is also used to appoint a guardian of a minor or incapacitated adult’s estate. The document is required by the Arizona Probate Code, and outlines the terms and conditions of the guardianship. It must be signed by the court, the guardian, the minor or incapacitated adult, and the minor or incapacitated adult's attorney. There are two types of Arizona General Order to Guardian and Acknowledgement: the General Order to Appoint Guardian and the General Order to Appoint Guardian of the Estate. The General Order to Appoint Guardian is used to appoint a guardian for a minor or incapacitated adult, and the General Order to Appoint Guardian of the Estate is used to appoint a guardian to manage and protect the assets of the minor or incapacitated adult. Both documents require the approval of the court, the guardian, the minor or incapacitated adult, and the minor or incapacitated adult's attorney.
