The Arizona General Order to Guardian and Conservator and Acknowledgement is a legal document used for appointing a guardian and/or conservator of an incapacitated adult in the state of Arizona. It includes the name and address of the adult in need of guardianship and/or conservatorship, the name and address of the proposed guardian or conservator, and the authority given to each appointed individual. This document must be signed by both the guardian and/or conservator and the incapacitated adult or the adult's attorney, and witnessed and notarized. There are two types of Arizona General Order to Guardian and Conservator and Acknowledgement: one for guardianship and another for conservatorship. The guardianship document grants the guardian the authority to make decisions and act on behalf of the incapacitated adult in matters such as their health, safety, and finances. The conservatorship document grants the conservator the authority to manage the incapacitated adult's finances, including their assets, investments, and debts.
