Arizona Voluntary Acknowledgment of Paternity (AVP) is a legal document that is used to establish the paternity of a child born to unmarried parents. It is signed by both parents, and upon the filing of the form with the Arizona Office of Vital Records, the father's name is added to the child's birth certificate, and he assumes legal responsibility for the child. There are two types of AVP: AVP in Person and AVP by Mail. AVP in Person must be completed at the Vital Records Office in Phoenix, Arizona, and requires the parents to be present and provide valid identification. AVP by Mail can be completed without both parents being present, but requires additional documentation to be submitted along with the form. Both types of AVP require the signatures of both parents and must be signed in front of a Notary Public or other authorized individual. Once the AVP is completed, the father's name is added to the child's birth certificate, and he assumes legal responsibility for the child.
