California Application to be Relieved as Attorney on Completion of Limited Scope Representation (also known as Limited Scope Representation (SSR) Relief) is a form used when an attorney has completed a limited scope representation in California. It is a written request from the attorney to the court to relieve the attorney from a case in which the attorney was only involved in limited representation. The California Application to be Relieved as Attorney on Completion of Limited Scope Representation must be filed in the court in which the case was heard. The form must include the attorney’s name, address, phone number, and bar number; the name of the client; the type of case; the date the representation was completed; the date the application was filed; a statement that the attorney has completed the limited scope representation; a request to be relieved from the case; and an acknowledgement that the client was informed of the application. The two types of California Application to be Relieved as Attorney on Completion of Limited Scope Representation are: 1. Application for Relief from Limited Scope Representation (Form MC-050): This form is used by attorneys who have completed a limited scope representation in California and wish to be relieved from the case. 2. Application for Relief from Limited Scope Representation (Form MC-050A): This form is used by attorneys who have completed a limited scope representation in California and wish to be relieved from the case but wish to remain as counsel of record.
