California Response to Request for Release of Minor's Confidential Information — Domestic Violence is a form used to obtain confidential information that relates to a minor's involvement in a domestic violence incident. This form is used to request the release of confidential information from law enforcement, courts, social service providers, and other agencies that may have records about the incident. There are two types of California Response to Request for Release of Minor's Confidential Information — Domestic Violence: (1) for individuals over the age of 18 who are requesting information about themselves and (2) for individuals under the age of 18 who are requesting information about their minor children. Both forms require the requestor to provide information about the minor, including the name, address, date of birth, and, if applicable, the name of the domestic violence incident. The requestor must also provide information about the requested information and the purpose for which the information is being sought. Upon submission of the form, the agency or agencies from which the confidential information is requested will review the request and determine whether the release of such information is appropriate in accordance with California state law.
