The minor must also provide a signature from a parent or guardian, indicating their consent to the application. The DMV offers two types of Emancipated Minor's Application: 1. The Minor’s Application for a Driver License or Identification Card (DL 44) — This form is used by emancipated minors who are applying for a driver license or ID card. 2. The Minor’s Application for a Certificate of Title or Registration (REG 256A) — This form is used by emancipated minors who are applying for a Certificate of Title or Registration for a motor vehicle. In addition to the forms, the minor must also provide any additional documents requested by the DMV, such as proof of identity, proof of residency, and proof of emancipation. Upon completion of the application, the minor must pay the applicable fees. Once the application is processed, the minor will be issued a driver license or ID card, or a Certificate of Title or Registration.
