The California Notice of Activation of Military Service and Deployment and Request to Modify a Support Order is a form used by service members in the military who are being deployed to modify their child or spousal support orders. This form is filed with the court when a service member is activated for military service and is being deployed. It informs the court of the service member’s deployment dates, the length of deployment, and the service member’s current address. There are two types of California Notice of Activation of Military Service and Deployment and Request to Modify a Support Order. The first type is the Request to Modify a Support Order Due to Military Deployment form. This form is used to notify the court of the service member’s deployment dates and to request that the court modify the existing support order due to the service member’s deployment. The second type is the Declaration of Intent to Modify a Support Order Due to Military Deployment form. This form is used to notify the court of the service member’s intent to modify the existing support order due to the service member’s deployment. Both forms must be filed with the court prior to the service member’s deployment.
