The California Information Sheet — Request for Determination of Support Arrears — Governmental (CIS-G) is a document used by the California Department of Child Support Services (CSS) to determine and collect arbitrages owed on child support and spousal support. The CIS-G is typically used in cases where the arbitrage has been resulted from the nonpayment of an existing court order. The CIS-G is divided into three different types: Type I, Type II, and Type III. Type I is used to requesting a determination of arbitrages when the CSS has a court order or a valid agreement between both parties. This type of CIS-G must be signed by both parties or their representatives. Type II is used to request a determination of arbitrages when the CSS does not have a court order or a valid agreement between both parties. In this case, the CIS-G must be signed by the party requesting the arbitrage determination or their representative. Type III is used to request a determination of arbitrages when there is a court order that includes an income assignment order. This type of CIS-G must be signed by both parties or their representatives. The CIS-G is used to provide information to the CSS to make a determination of arbitrage owed and to establish a payment plan if necessary. The information requested on the CIS-G includes the names of the parties involved, their addresses, the date of the court order, the amount of the court order, and other relevant information. The CIS-G also requests information regarding any payments made, any payments that have not been received, and any relevant payments or credits that have not been applied to the arbitrage. Once the CIS-G is completed and signed, it must be filed with the CSS for a determination of arrears.
