The California Qualified Medical Evaluator (ME) and Agreed Medical Evaluator (AME) Time Frame Extension Request is a form designed to extend the time frame for completing a medical evaluation from the required 30-day period. The California BECAME Time Frame Extension Request is used when the ME or AME doctor needs more time to complete the medical evaluation due to certain reasons, such as an unanticipated event or an unexpected delay. The request must be made within the 30-day period, and the request must be approved by the Division of Workers’ Compensation (DWC) before the extension can be granted. There are two types of California BECAME Time Frame Extension Request: a one-time extension request and a re-evaluation extension request. The one-time extension request is for an initial medical evaluation, while the re-evaluation extension request is for a subsequent medical evaluation. The form must be completed and filed with the DWC in order to be approved.
